Below is a listing of our telephone extensions. Calls are returned same day if possible. Our telephone hours are from 8 a.m. to 3 p.m. on Mondays and 7 a.m. to 1:30 p.m. on Tuesdays through Fridays.
Medical Questions: Extension 108
New Patient Appointments: Extension 108
Return Appointments: Extension 100
Medication Refills: For prescription refill requests, please ask your pharmacy to fax a request to us at 744-1654 or send us an electronic notification. If you are using a new pharmacy enter extension 108.
Billing Questions: Extension 107
Medical Records Requests: Extension 107. There is a $6.50 charge for paper records sent directly to a patient. There is no charge to send your records to another doctor’s office. |
